This 6-month Medical Planner & Journal is for anyone who is about to battle an illness or for the care taker that will be assisting the patient. Battling an illness can be very stressful and overwhelming. I know this personally because I was diagnosed with Breast Cancer at the age of 35 and Metastatic Breast Cancer in the Bone at the age of 37. Before treatment begin, during treatment, and after treatment there will be many doctor appointments and this planner is here to help you stay organized. In the download, there is also a journal to document your inner thoughts and feelings.

Prescription Tracker: This tracker is used to note the various medications that have been prescribed to you by your physicians. Each doctor you see will want to know the name of the medications you are taking, the dosage, and the frequency. There is also a note section for medications that you may be allergic to or for you to note side effects that you would like to discuss with your physician.
Surgical History: Depending on your particular illness, there may be several surgical procedures. For a Breast Cancer patient, you may have a Lumpectomy, Mastectomy, or maybe a Port installed. After all of your treatments are complete, future physicians will ask for your surgical history. Having them all listed with dates will keep you from having the daunting task of trying to remember all of them.
Scan History: During your course of treatment, you may have multiple scans such as: CT Scans, MRI, PET Scans, etc. Each time you go for a scan, the physician will ask for your scan history. They want to know what scans you have had and when. They will ask for this information after your treatments end as well. Note: It is always helpful to bring the disk of the last scan you had to your current scan appointment if it is not performed at the same location.
6-Month Weekly Calendar: The weekly calendar is undated so that you can utilize the calendar at any time. The weekly calendar can be used to note all of your doctor appointments, scans, surgeries as well as all of your other daily activities.
6-Month Monthly Calendar: The monthly calendar is undated.
Journal: There is a journal at the back of the planner. It can be used to document your journey, a space to free your thoughts and feelings, or used for additional space to take notes from your doctor appointments.
